WAIVER AND RELEASE OF LIABILITY

I, {name}, agree to participate in activities at RebelFire MMA, operated by 1492869 B.C. LTD., located at C108-19770 94A Avenue, Langley, BC, V1M 3B7 ("the Gym").


1. Acknowledgement of Risks

I understand that participating in Brazilian Jiu Jitsu, Kickboxing, Mixed Martial Arts, fitness classes, open mat sessions, and other activities at the Gym involves significant physical risk, including the risk of serious injury, disability, or death.
I voluntarily accept and assume all risks, whether foreseeable or not, including those arising from the negligence of the Gym, its owners, instructors, staff, volunteers, or other participants.


2. Waiver and Release

In consideration of being allowed to participate, I hereby waive, release, and discharge the Gym, its owners, instructors, staff, volunteers, and affiliates from any and all liability for any injury, death, or damage to personal property arising from or in connection with my participation — even if caused by negligence.


3. Indemnification

I agree to indemnify and hold harmless the Gym, its owners, and its affiliates from any claims, damages, liabilities, or expenses (including legal fees) incurred as a result of my participation.


4. Medical Authorization

I acknowledge that I am physically fit for participation and understand that it is my responsibility to consult a physician prior to engaging in physical activity.
In the event of injury or illness, I authorize the Gym to obtain emergency medical treatment on my behalf if deemed necessary.


5. Media Release

I grant permission to the Gym to take and use photographs and video recordings of me for promotional purposes, including but not limited to websites, social media, and marketing materials. I waive any right to compensation for the use of such media.


6. Severability

If any portion of this Waiver is found to be invalid or unenforceable, the remaining portions will continue in full force and effect.


7. Governing Law

This agreement is governed by the laws of the Province of British Columbia.
Any disputes will be resolved in the courts of British Columbia.


I HAVE READ THIS WAIVER, UNDERSTAND IT FULLY, AND SIGN IT VOLUNTARILY, KNOWING THAT I AM WAIVING IMPORTANT LEGAL RIGHTS.
